More detail about SDS
Self Directed Support (SDS) is a new scheme which provides individual budgets to people, so they can plan and manage their own care. This gives you more say about the care you want.

Who is eligible?
To receive SDS, you will need to be assessed by your local authority on your health, personal and social care needs. They assess your ability to live safely at home - particularly relevant if you are recovering from ill health or operations including strokes, dementia or alzheimer's.

Establishing a budget and making a care plan
If you are eligible, you must complete an Individual Budget Assessment Form which enables your Local Authority to establish how much your budget needs to be.

You will also need to provide a ‘support plan’ which outlines the care you need and how it will be managed. Bright Yellow can help you through this process. All plans must be approved by your local authority.

You are free to hold and manage your own budget, or you may ask for help from others such as Bright Yellow who can do this for you. Your Local Authority keeps track of your progress along the way.
